CITY Connections (Creating Individualized Transitions for Youth) is a community based program for students with disabilities age 18-21. Each student has an individually designed program based on goals identified by the student, families and educational staff members. The program provides an opportunity for young adults with disabilities to continue their public school educational program in an urban, community-based environment outside of a traditional high school campus. Students attend the program at an apartment site or college campus. The students with special needs receive their education in a natural, age appropriate setting with access to real life and real work in the community. All students receive programming related to outcomes in post-secondary training, employment, community living and recreation/leisure. Students prepare to live, work, connect and contribute in the community.
